Small Hospital Improvement Program Grants (SHIP)
Every year HRSA's Office of Rural Health Policy offers grant programs to every state, where they can receive roughly $8,500 per rural hospital under 50 beds. The WI-ORH applies for this money each year, asking for applications from all qualifying Wisconsin hospitals at the beginning of the calendar year. The grant year begins Sept 1st of each year, with funded activities completed by Aug 31st of the following year. Progress reports and documentation of expenses must be submitted by each hospital after the grant year ends.

Hospital Acquired Conditions/Readmissions Reduction Grants
The Office of Rural Health will award up to $5,000 to rural hospitals under 50 (staffed) beds to design and implement programs to reduce avoidable readmissions and/or hospital acquired conditions. Award selection will be based in part on the applicant hospital’s quality indicator as reported in iVantage’s Hospital Strength Index (TM). The deadline for the 2014-15 proposals has ended.

Rural Community Grants
The Rural Community Grant Program provides funding to rural coalitions who are addressing a health care issue in their community. Eligible coalitions must include a Wisconsin rural hospital with fewer than 50 beds.

How to Apply
Guidance for the 2015-16 grant year should be released in April/May of 2015, with a deadline in July of 2015. In the meantime, please familiarize yourself with the last grant guidance and application:
